GerI

GerI: educational resource for geriatricians and physicians seeing older people

An educational resource for geriatricians and physicians seeing older people, starting with ACMI Scanner: anticholinergic burden and deprescribing prompts from a photo of the medication list.

GerI is a toolkit for registered healthcare professionals. ACMI Scanner is its first tool; a STOPP scanner for all medicines and a START scanner are next. Point the camera at a repeat list. Get the Anticholinergic Medication Index, the STOPP v3 criteria to consider for each contributing drug, and a deprescribing plan with a downloadable report. Everything runs on the phone.

Camera, a saved photo, or paste text. Text recognition runs on the device.

1

ACMI

Score, category and a one-line summary for the notes. Brand matches flagged for you to confirm.

2

Refine

Optional clinical context tick boxes and an end-of-life mode that separates symptom-control medicines.

3

STOPP

STOPP v3 criteria for each contributing drug, collapsed per medicine, with a tick box to consider deprescribing.

4

Plan

Taper, cautions and monitoring prompts, ACMI before and after, and a downloadable report.

What you get in the notes

One line, computed in code, never by a language model.

ACMI: morphine 1.5 + oxybutynin 1.2 + furosemide 1.1 + amitriptyline 1.0 = 4.8 (High)

88-drug ACMI

Generic matching with UK brand and combination-product mapping. Each ingredient scores once; PRN only if every entry is PRN. Topical preparations and Buscopan are never scored.

Reads real lists

Uses only the current/repeat section when an acute section is present, ignores stopped items, and merges direction lines with the medicine above.

STOPP v3 built in

Criterion text verbatim from O'Mahony et al. 2023. List-level rules (M1, A3, L2, L3) and route-dependent advice for inhaled versus systemic glycopyrronium.

End-of-life mode

Mark symptom-control medicines; their criteria collapse and everything else is flagged for review, rather than hiding all prompts.

Deprescribing plan

Generic taper, cautions and "restart if" prompts per drug class, with drug-specific notes such as furosemide in heart failure.

Private by design

No account, no network calls, no stored photos or text. Only anonymous usage counters, which you choose whether to share.

Download

Evaluation release for registered healthcare professionals. GerI version 2.0, containing ACMI Scanner. Testing is by invitation: creating an account needs an nhs.net email address and the invite code from your GerI leaflet. No leaflet yet? Email the address below to ask for one.

Android

Signed APK, about 15 MB, Android 8 or later (arm64).

Download APK

Open the file on the phone and allow installs from your browser or Files app when asked. Samsung phones: switch Auto Blocker off first (Settings, Security and privacy). Updating from ACMI Scanner 1.x: GerI installs as a new app, so uninstall the old ACMI Scanner afterwards; sign in with the account you already have. A Google Play listing will follow.

iPhone

TestFlight beta coming soon. Register interest and you will receive the TestFlight link when it opens.

Register interest

Requires iOS 16 or later. The Apple Watch companion app lets you dictate drug names for a quick score.

Decision support only. GerI and its tools do not provide medical advice and does not replace clinical judgement or the full medication record. It is intended for registered healthcare professional prescribers and is currently an evaluation release, not a regulated medical device. Verify every result against the source list before acting.

Try it on this sample list (expected result: 7.3, High). Read the privacy notice or manage your account.
